Approach: Infra-Mammary Submuscular - Revision/Re-do
Implants: Mentor Round Smooth 425 cc Saline Filled
Volumes: 425cc Right/ 450cc Left
Outcome: 34 "D" cup
Photo Date: 3 months post-op
Comment: Patient came to me 5-6 years post augmentation with textured saline implants (225ccRight/250ccLeft) with significant "bottoming out" of both implants, but Left much greater than Right, due to breach of inferior muscle support fascia. This caused both nipples to show above bra or bathing suit line and was accompanied by significant firmness of breasts due to implant scar capsule contractures. Correction was done via inframammary approach, raising the inframammary fold 3cm on the Left and 1.5 cm on the Right, along with capsule release and excision. Implant size was almost doubled as well, with split volumes to correct pre-existing volume asymmetry. Last photos show the extent to which I like to maintain pocket patency post-op to keep the implants soft, freely mobile, and natural to the touch and eye.
